RN- Hospital Care Manager- Up to $$74.30/ hour Interim Health Care Staffing is currently hiring hospital RN Care Managers for immediate needs in premier local hospital! Schedule: Full-Time, M-F 8am - 4:30pm Pay rate: Up to $74.20 depending on experience Position Responsibilities Include:

• Conducts thorough and accurate psycho-social assessments of patients in an inpatient hospital unit in a timely manner with a patient centered approach.
• Actively participates on interdisciplinary team to develop and implement care plans during hospitalization.
• Establishes and implements care plans to ensure safety, continuity of care, appropriate care coordination and support post hospitalization.
• Uses knowledge of community resources, healthcare funding, along with county, state, and federal regulations in making appropriate and safe discharge plan.
• Communicates promptly, clearly, respectfully and effectively with care team, patient and support system. Coordinates and attends care conferences as applicable.
• Documents work clearly, concisely and consistently in a timely manner.
• Works closely with the Social Worker throughout hospitalization and during discharge planning.

Required Skills/Qualifications:

• An active RN license in the states of MN.
• BSN is preferred but not required. years of current RN experience is preferred.
• One year of hospital Case Management experience is preferred.
• Current BLS/ CPR
• Ability to pass a criminal background check.
