Position title: Outreach Counselor
Department: Counseling
Location:   Lyons Township Office . -La Grange, IL 60525
Schedule: Flexible Hours/ Mon to Thursday 12 pm to 8 pm and Friday 9 am to 5 pm 

Are you passionate about working with at-risk youth and helping them build brighter futures? Youth Outreach Services (YOS) is looking for a dedicated and compassionate  Outreach Counselor to join our team.

Outreach Counselor Key Responsibilities:

• Conduct outreach to engage high-risk youth and their families in services.
• Meet with parents/guardians to support youth engagement.
• Provide counseling, employment, educational, and life skills training.
• Coordinate with YOS services to connect youth and families to resources.
• Offer transportation to youth as needed.
• Build relationships with referral sources that are beneficial to clients.
• Make referrals to appropriate internal and external programs, ensuring timely follow-ups.
• Develop and implement service plans tailored to youth needs.
• Maintain accurate documentation in electronic records.
• Participate in team meetings, supervision, and collaborative agency programs.

Outreach Counselor Qualifications:

• Bachelor's degree in Social Work, Counseling, or Psychology (required).
• Experience working with at-risk youth and families.
• Bilingual in Spanish. 
• Familiarity with outreach programs.
• Valid driver's license and reliable transportation with proof of insurance.
